Individuals with HIV/AIDS who reside or receives health services in Albany, Clinton, Columbia, Delaware, Essex, Franklin, Fulton, Greene, Hamilton, Montgomery, Otsego, Rensselaer, Saratoga, Schenectady, Schoharie, Warren, and Washington counties

Required Documentation
Verification of HIV positive status, ROI, signed consent and client agreement forms, intake assessment; call for information
