Required Documentation
Applicants must have a denial letter from the Department of Social Services; if applying for utility assistance: applicant must have a disconnect notice if applying for rent/mortgage assistance: applicant must provide proof of their current status (behind in rent/mortgage, pending agreement to rent, etc.), proof of income, birth certificate, driver's license, proof of residency, utility bill with address are required
